Participating Providers: BuckCard.com provides various products and services, often referred on this website as “programs” or “benefits.” Some benefits require you to choose a provider from a directory of participating providers. The term “provider” refers to an individual or practice agreeing to participate in a particular plan or benefit. . Before signing up for any benefit that requires you to choose a provider in your area, make sure a provider exists convenient to your location. Not all areas have providers in a reasonable distance.

Additional Information May Be Required: Some benefits that you purchase through BuckCard.com do not become active until you complete additional information and provide it vendor. For example, if you choose to participate in the 1800MD telemedicine benefit, you must first complete additional information online, such as providing your prior medical history. As another example, after you have purchased LifeLock® identity theft protection, your protection is not activated until you provide additional, confidential information to LifeLock®.

Payment Due Pending Additional Information: Your payment is due to BuckCard, even in the period between the time you sign up and the time you complete the information, so please complete the information as soon as possible. Instructions for completing the information are provided in the membership kit.

Membership Kits: If you do not receive the membership kit, it is your responsibility to call 800-800-7616. Also note that your membership includes the original instruction booklet and membership cards. If you need additional or replacement cards, or a replacement instruction booklet, there is a nominal fee that you will need to pay by credit card prior to new cards being issued. Call us at 800-800-7616 for replacements—these cannot be ordered online or by email.
